Interim: Transitions of Care (TOC) Nurse

West Coast

Solutions to Transform Care can be found in the Sunshine State!

Interim details:

• Basic knowledge of computers required.
• Able to work remotely, required.
• 3–4-month Interim assignment position to begin with.  Position could go permanent, but there is NO guarantee that this will occur.  
• Must be a current resident of California, required.

Salary: $45.00-50.00 per/hour

The Ideal candidate will be:

• Bachelor’s degree in nursing, required.
• Unrestricted California registered nurse license, required.
• 3+ years of clinical nursing experience as a nurse in a hospital, required.
• 1-2+ years case management, utilization review or discharge Planning experience in an Acute care or home Health setting, required.
• Recent experience working with Medi-Cal and/or Medicare populations or any other state/federally sponsored programs, required.
• Experience working in a managed care setting required.
• Valid California driver license and reliable transportation required.
