About the Position: Not-for-profit, general care medical facility as well as a state-of-the-art acute care unit. It offers to the community, Cardiac/Cancer Centers, a Birthing Center, and Geriatric Psychiatry program. This particular hospital has partnered with the community to provide free, low cost, classes, and support to various groups. They are committed to being part of the community and finding ways for the community to be part of the hospital.


About the Position: Responsible for oversight of the Case Management staff and the day-to-day activities of the department. Duties will include analyzing patient records, working with staff to plan for patient discharge and the development of the Case Management staff skills through training and education. Reports to the Director of Home Health who has received State recognition as an outstanding leader.


Type of Work: Full time, Onsite



  • BSN,
  • Master’s in nursing or related field,
  • Current state RN License,
  • Certified Case Manager (CCM),
  • 2+ years of leadership experience managing a Case Management department,
  • 5 years of case management experience in a healthcare setting,
  • Strong knowledge of Case Management, Utilization Review and Discharge Planning,
  • Knowledge of community health and social service resources,
  • Experience planning and managing the annual budget for the department, r
  • Willingness to be Hands-on, as needed depending on staff levels and case load, required
