Nurse Case Manager – Long Term Care 

(Managed Care)

Position:   

Join a California Healthcare organization looking for a Nurse Case Manager who specializes on long term Care cases to perform case management activities on assigned cases where analysis, synthesis and evaluation of data are required. Responsible for developing, implementing, and evaluating individual care plans to ensure that care is rendered at the most appropriate level, for the requisite length of time, and in compliance with the organizations, regulatory and contractual requirements. Work is varied and complex and requires a moderate amount of discretion and independent judgement. This position works closely with and reports to the Manager of Case Management.  

Position Type/Expected schedule:  

• Hybrid work environment: Remote/office   
• Monday-Friday work schedule  

Qualifications:   

• Bachelor’s degree in nursing, required.  
• Unrestricted California Registered Nurse license, required.  
• Valid California driver license and reliable transportation required.  
• Certified Case Manager Certification, preferred.  
• 3-5+ years clinical experience as a nurse in an acute care setting, required  
• 3+ years of recent experience working in case management, utilization management or discharge planning, required. 
• Recent experience as a clinician in a managed care setting, preferred.  
• Experience working with Medi-Cal and/or Medicare populations, strongly preferred.  
• General knowledge of NCQA Standards, preferred.
