Physical Therapist – Home Health

Join a talent team of healthcare professionals at a small network of clinics and a thriving community hospital as their Home Health Physical Therapist. The Physical Therapist will evaluate the care plans and administer comprehensive physical therapy modalities/regimens to patients in their place of residence for the purpose of developing and restoring function as prescribed by a physician. Will coordinate care planning and delivery with the physician, patient and family/caregivers, other healthcare team members and HHA staff to facilitate optimal patient outcomes. Assesses the patient for the need for durable medical equipment as well as other adaptive equipment needs. Develops appropriate home exercise programs tailored to the patient’s clinical status, needs and age. This position reports directly to the Rehabilitation Services Director.

This hospital would like to find a Physical Therapist with:

• Bachelor’s Degree in physical therapy,
• Doctor of Physical Therapy,
• Current state physical therapy license,
• BLS Certification,
• Must have a valid state driving license,
• Knowledgeable and adherence to all applicable laws, regulations, and standards state and federal
