This 190+ bed Magnet Hospital is located on the north side of Dallas/Fort Worth Metroplex in a rapidly expanding area that has been named one of “The Best Places to Live in America for Families.” They offer advanced medical services for women’s services, neonatal intensive care, orthopedics, heart and vascular, neurosciences, stroke, diagnostic imaging, intensive and emergency care.

 

The Director of Case Management is responsible for planning, developing, and implementing the continuum of care and case management functions, which may include social services, coordination of patient care, patient access, utilization management, and discharge planning. In addition, this person will have oversite of palliative care, regulatory and compliance, patient safety and quality, or other patient care support activities.

 

This Director will have 17 FTEs who are a mix of RNs and Social Workers. The Director will report to the Regional Director of Case Management, with a dotted line to internal senior leadership. The Regional Director of Case Management has been with the system for 13.5 years! She is described as a servant leader who believes in inspiring others through innovation and evidence.

Requirements:

  • Bachelor’s degree in nursing, social work, or related field preferred. Master’s degree preferred.
  • 3+ years of experience in acute care hospital case management, social work, or utilization review required.
    • Director level experience required.
  • Registered Nurse (RN) or Licensed Master Social Worker (LMSW) required.
  • Knowledge of hospital case management, discharge planning, transition of care, transfer coordination.
  • Excellent verbal, written, interpersonal, and presentation skills.
  • Ability to lead and influence multi-disciplinary teams in the delivery of a comprehensive care program.
