Type of Work: Full time, Hybrid (remote/onsite) within the state of California


Join a California Healthcare organization looking for a Director of Case Management to oversee the Case Management and Patient Health Navigators.     Ensure that the level of quality, cost effective services are medically necessary, contractual benefits are in regulatory compliance for NCQA and within the health organization objectives.   The Director collaborates with interdepartmental units to establish and improve workflow and processes; identifies issues and opportunities; resolves issues or makes recommendations as required.  Develops and manages department budgets; implements appropriate interventions.  Uses strong oral and written communication skills and can communicate professionally, effectively and persuasively to diverse individuals and groups inside and outside the organization.  This position works closely with and reports to the Executive Director of Clinic Operations.


The Ideal candidate will be:

  • Bachelor’s degree in nursing, required
  • Master’s degree in nursing,
  • Unrestricted California Registered Nurse license, required
  • Valid California driver license and reliable transportation, required
  • Strong working knowledge of NCQA Standards required.
  • Certified Case Manager Certification, required
  • 5+ years clinical experience as a nurse in an acute care setting, required
  • 5+ years of recent experience working in case management, disease Management and/or Quality improvement in a managed care setting, required
  • 5+ years of progressive leadership experience in a healthcare setting, required.
  • Experience working with Medi-Cal and/or Medicare populations, strongly preferred
